Your child’s spot will be confirmed only after all three steps below are completed.

Please complete the online registration form during the registration period.

2. Consent Forms (Two Required)
All participating children are required to submit two consent forms.
Please download both consent forms from the link provided, complete them, and email the signed forms to:
With approval from Aliʻi Animal Hospital,
MIRAI HANA will collect and manage all consent forms on their behalf.
If you have any questions regarding the content of the consent forms,
please contact Aliʻi Animal Hospital directly.
3. Payment (Sold out)
Once registration opens, please complete payment using the link below.

Spots are available on a first-come, first-served basis.
Registration is not complete until the registration form, consent forms, and payment are all received.
Session changes (AM / PM) are not possible after payment is completed. Please select your session carefully.
