This form is required for your child to participate in the Keiki Market event.
It includes a liability waiver, so please read each section carefully and check the boxes to indicate your consent.

⚠️ Please Note

• Your child’s seller spot will be secured only after both the registration form and payment are completed.

• Some information may be edited after submission; however,
  no changes will be accepted after Friday, March 27, 2026.

• You may edit your responses directly through the confirmation copy sent to your registered email address.

👉 Please submit one form per booth application.
👉 This form is for sellers only.
👉 A separate form for buyers will be available at a later date.

Booth Assignment

※ Booth locations will be assigned by lottery on the day of the event.
If you wish to be next to a friend, you are welcome to coordinate and arrange a swap directly with other participants after booth assignments are announced.

If a seller registration is canceled, all related priority reservations and seller perks
(including Mini Kids Job Workshops and Easter activities) will also be canceled automatically.
